Parent's Instructions for School and Child Care

Free family safety plan letter: names the people you trust to pick up and care for your children if you cannot be reached. When a California public school or licensed child care facility knows a parent is unavailable, it must first use the parent's instructions in the child's emergency contact information — so put the same names on the school's emergency card. Private details (A-number, consulate, where documents are kept) go on a separate family-only Caregiver Sheet, never on the letter.

Consent for Minor Child to Live with Non-Parent

The California courts' self-help sample form: you allow your children to live with a caregiver and check what the caregiver may do — medical care, school, travel, benefits. Parents sign before a notary; the caregiver signs too. It does not give custody, and you can cancel it at any time.

Authorization for Consent to Medical Treatment

Allows agent to authorize any medical care for the child.

HIPAA & FERPA Authorization

Grants access to medical and educational records.

Caregiver's Authorization Affidavit

Official CA form to enroll a child in school and obtain medical care. Signed by the caregiver once the child lives with them.

Nomination of Guardian

Tells a court whom you would want as your child's guardian; the court decides.

Durable Power of Attorney for Financial Matters

Manages finances, pays bills, and handles property. California's statutory form changes on January 1, 2027 (AB 2199): if you prepare it here before then and cannot sign by December 31, 2026, we prepare the new form at no charge (it adds a few optional choices).

Child International Travel Consent Letter

Parental consent letter for a minor child traveling internationally without one or both parents. Strongly recommended by U.S. Customs and Border Protection (CBP); some destination countries and airlines may independently require it (often notarized or apostilled). Per-trip document; covers one or more children in the same family.
